There's a lot going on in this cartoon. What the hell does it mean?

To Mahnkenstein: Sheila Jackson was on her cell phone during one of her recent townhall meetings on healthcare. No one has figured out who she was talking to, but there was humorous speculation that she was getting "advice" on how to handle the "astroturfed" crowd. During Obama's inauguration (I think), Chris Matthews said he got "this thrill, running up [his] leg". The rattle snake is one of the earliest symbols of America freedom, in this case a reference to the "Don't tread on me" flag, which you can easily find via google. Hope that helped!
